Tip of the Month:

Every nursing facility care plan must include FOUR Elements in order to be fully compliant with the expectations of Iowa HHS, Maximus, and the Department of Inspections, Appeals, and Licensing (DIAL).  If facilities use the Iowa PASRR LII PASRR Care Planning Tool, as intended, these elements have all been incorporated and the care plan will be compliant.  If the Iowa tool is not used, the facility must make sure that each unique service appears on a separate line of the care plan and the FOUR elements are included for EACH SERVICE in all of their care plans/tools/templates.  The  FOUR required elements of a compliant care plan include, in addition to identification of the specific service itself:

  1. The name of the provider of each service and their professional 
  2. The specific/exact Start date of the particular service
  3.  The anticipated frequency for the particular services
  4. The expected duration of the service (or when it will be reviewed next to determine whether it continues to be appropriate/needed) .
