How To: Set up an Access Coordinator for your facility

Tip of the Month:

It is essential that the potential receiving facility review and understand the PASRR Level II Summary of Findings prior to accepting any individual with a PASRR condition to the nursing facility. By accepting the person, you are making a commitment to deliver 100% of the PASRR identified services. Each service must be added to the PASRR compliant care plan, and each must be specifically addressed and delivered.

We remind providers that, pursuant to the Olmstead Supreme Court Decision, all individuals with disabilities have a right to reside at the least restrictive, most integrated setting that is appropriate to their needs and choices, so all individuals that are served in a nursing facility, regardless of PASRR status, must have a care plan that addresses proactive discharge planning if/when the individual expresses a desire to make efforts toward placement at a lower level of care.
