Nursing Facilities must always make entries of admission, discharge, transfer, and corrective notices within ten days of the event, (ideally within 2 days of the event), submitting those into the PathTracker Plus functions of the PASRR database. Nursing facilities must never utilize/complete paper Case Activity Report forms (CAR forms), even if asked to do so by HHS Income Maintenance staff, and please make certain that your business office is not sending in duplicate forms, as ALL changes must be made in PathTracker Plus. Hospitals do not use PathTracker Plus and typically don't need to attend training on PathTracker. Other levels of care, hospice, and other non nursing facility programs do continue to use paper CAR forms for submission of admission/discharge/transfer data.
