Tip of the Month:

The receiving nursing facility is responsible for tracking the length of any short-term approvals and upcoming expiration dates for any/all PASRR Level of Care approvals.  An expiration date on any PASRR creates an expectation that  significant and proactive efforts will be made to achieve successful discharge to a lower level of care on or before the expiration of PASRR approval for nursing facility level of care.  If discharge is not achieved by the end of the period of approval, the facility must send in a new Level I Screen SEVEN - TEN (7 to 10) days prior to the expiration date to permit enough time to have a new approval in place by the expiration date, if appropriate.  Facilities should share, at the time of Level I screen submission, and in the interview process for any new LII summary of findings, why their proactive efforts toward service delivery and discharge planning have not yet resulted in successful discharge to a lower level of care.                                                                                                                                                                                                                                                                We remind providers that, pursuant to the Olmstead Supreme Court Decision, all individuals with disabilities have a right to reside at the least restrictive, most integrated setting that is appropriate to their needs and choices, so ALL individuals that are served in a nursing facility, regardless of PASRR status, must have a care plan that addresses proactive discharge planning if/when the individual expresses a desire to make efforts toward placement at a lower level of care.
