We have contracted with the State of Illinois Department of Healthcare and Family Services (HFS) to manage the assessment process for Preadmission Screening and Resident Review (PASRR) Level I screens and completing Level II Serious Mental Illness (SMI) evaluations for individuals applying to, or residing in, Medicaid-certified nursing facilities. Other services include Follow Up Visits, admissions support for Specialized Mental Health Rehabilitation Facilities (SMHRF) and Supportive Living Programs (SLP) settings and Colbert Dementia Review. We have contracted with the State of Illinois Department of Healthcare and Family Services (HFS) to provide online Preadmission Screening and Resident Review (PASRR) Level I screening, online Level of Care utilization review for long-term care populations, and onsite, independent PASRR Level II mental health and Level of Care evaluations.

Follow Up Visits
We have contracted with the State of Illinois Department of Healthcare and Family Services (HFS) to manage the assessment process for Follow Up Visits, an in-person visit scheduled between the 30th and 60th day of a person's Nursing Facility (NF) stay who has previously received a Preadmission Screening and Resident Review (PASRR) Level II Mental Health Assessment.

Outreach and Transitional Assessment
We have contracted with the State of Illinois Department of Healthcare and Family Services (HFS) to conduct outreach visits that provide education to class members about their rights as Williams and Colbert Consent Decree Class Members. During the interview, the class member will have the opportunity to express interest in potential transition to the community. For those interested in transition, a needs assessment will be conducted. Their functional needs will be evaluated in several areas to determine the level of services necessary for successful community transition.
