Maximus has been contracted to review all Pre-Admission Screening and Resident Review (PASRR) positive Level I screens for all individuals who are applying to or residing in Medicaid-certified nursing homes and complete Level II evaluations for individuals confirmed or suspected to have mental illness. This federally mandated assessment process is used to determine whether the nursing home is the appropriate placement for individuals with mental illness, intellectual disabilities or developmental disabilities. PASRR functions as an essential method to help match individuals with the care and services they need.

  • South Dakota Pre-Admission Screening and Resident Review (PASRR) Program: Recent Management Transition Process Now Completed | Recent Management Transition Process Now Completed | Review Level I Process Reminders, FAQs and Helpful Training Resources

    We are now a few weeks past the recent transition of Pre-Admission Screening and Resident Review (PASRR) processes to management through state partner, Maximus. They have now begun review of PASRR positive Level I screens for all individuals who are applying to or residing in Medicaid-certified nursing facilities and completion of Level II evaluations for individuals confirmed or suspected to have mental illness. 

    The team at the South Dakota – Department of Social Services (DSS), Division of Medical Services and Maximus would like to share a sincere note of thanks for your continued support and assistance throughout this implementation. We look forward to continuing our partnership with you to ensure individuals across the state receive the support and resources they need.

    REMINDERS: Understanding the PASRR Level I Process

    Who is required to have a Level I Screen?
    All persons seeking entry into a Medicaid funded NF, regardless of payer source, diagnoses, or current location must be screened prior to NF admission. Although the Final Rule for PASRR does not require Level I Screens for people going to a swing bed, currently South Dakota Policies and Procedures do require that a Level I Screen be completed for people needing a swing bed (and Level II evaluation, as applicable). 

    Does it matter what time of day a Level I Screen is submitted? 
    Maximus will process all referrals received before 3 pm CT on that business day and the review will be completed by end of the following business day. Referrals received after 3 pm CT will not be processed until the following business day (and review completed by the end of the day after that day). 

    Will I hear from Maximus if I didn’t submit all of the required documentation with the Level I Screen?
    Yes. If all required documentation was not received with the Level I Screen, Maximus staff will contact you (up to 3 times) to request the documentation that was not received. If the required documentation is not received by the 10th business day, the screen will expire. 

    How long is a Level I outcome valid?
    The Level I outcome remains valid for 60 days on a delayed admission. If they discharge to the community, the outcome is no longer valid and a new Level I screen would need to be submitted.
     
    What is the Level I Process?
    PASRR Level I Screens need to be initiated on behalf of a person seeking NF admission, regardless of their payor source as soon as the need for a NF becomes evident. When completing the Level I Screen, if any of the answers for questions 1 through 3 are “YES”, or “Unknown” email Maximus at PASRR@state.sd.us with this screen and required supporting documentation. Maximus will review your Level I Screen and submitted documentation within 1 business day and determine if the individual does have a confirmed or suspected PASRR condition and if so, will indicate if a categorical determination applies or if the person needs a Level II evaluation. 

    We encourage providers to provide all required documentation upon submission. Your required documentation will depend on what type of screen you have completed. Please see the table below for the required documentation.

    • Copy of order for new diagnosis, medication, or for other identified status change reason

    Note: For any categorical decision, we may require additional documentation along with these required records. Please see question below regarding categoricals for the required additional documentation.


    FAQs: Exempted Hospital Discharge

    Q: What are acceptable exemptions from PASRR?

    A: The only true exemption from PASRR is the Exempted Hospital Discharge. This exemption may be applied for a person who is admitting to a NF directly from a hospital after receiving acute inpatient medical care at the hospital. The person must require NF services for the condition for which they received care for in the hospital. The attending physician must certify that the stay will likely require less than 30 days of NF services. The person must meet all these criteria to be eligible for an Exempted Hospital Discharge.
     
    Providers should send the completed SD Exempted Hospital Discharge Form to PASRR@state.sd.us and the nursing home. There are no other required documents, and a clinical review of these forms is not completed. Maximus simply reviews to ensure it is filled out properly and uploads to the state database. We do not provide an outcome.

    As shared in previous communications, next Monday, October 16, 2023, will mark the official management transition of Pre-admission Screening and Resident Review (PASRR) processes from South Dakota-based entities to Maximus. Selected earlier this year by the State of South Dakota – Department of Social Services (DSS), Division of Medical Services, Maximus will begin ongoing review of PASRR positive Level I screens for all individuals who are applying to or residing in Medicaid-certified nursing facilities and completion of Level II evaluations for individuals confirmed or suspected to have mental illness. 

    Please closely review the checklist items spotlighted below to prepare your team for these upcoming changes and avoid any delays in upcoming PASRR submissions.

    The State of South Dakota – Department of Social Services (DSS), Division of Medical Services and selected partner, Maximus continue to move toward the planned October 16 transition for Pre-Admission Screening and Resident Review (PASRR).     What’s changing? 
    Starting on October 16, the performance of various PASRR-related services currently performed by South Dakota-based entities will transition to Maximus.
