Louisiana Preadmission Screening and Resident Review (PASRR)

About the Program

The State of Louisiana Department of Health (LDH) - Office of Aging and Adult Services has contracted with Maximus to perform online Preadmission Screening and Resident Review (PASRR) Level I Screens for individuals who are applying to Medicaid-certified nursing facilities.

The primary goal of the federally mandated PASRR assessment is to determine whether the nursing home is the appropriate placement for individuals with mental illness (MI), intellectual disabilities (ID), developmental disabilities (DD), or related conditions (RC).

PASRR also works as a critical function to help match individuals with the care and services they need. Scroll down for Provider tools, frequently asked questions, training opportunities, and historical information.

Note: To open any training file links below (indicated with a “lock” icon), simply click the link and when prompted, enter your AssessmentPro system Username (your email address), Password and click Log In. You will then have access to all protected documents located on the Resources page for the remainder of your web session.

Tip of the month:

Double check all demographic information is correct before submitting a Level I screen, such as correct spelling of the first and last name, the SSN and DOB. Inaccuracies can result in duplication, wasted resources, inefficiencies, inconsistencies, and delays in receiving a 142.
