Indiana Level of Care Assessment Representative (LCAR) provides several types of services and assessments for Hoosiers receiving or seeking long-term care that help them navigate complex processes and ensure they get the appropriate supports they need.
Assessments
A Nursing Facility Level of Care (LOC) assessment is a way to measure care needs that must be met to go to a Medicaid nursing facility or to access long-term care services and supports from Home and Community Based Services (HCBS) waiver services. We provide assessments for several HCBS Waiver services available in Indiana for adults and children.
We also administer the Preadmission Screening and Resident Review (PASRR). This requirement for admission to any Medicaid-certified facility helps to make sure that people with disabilities receive care in the best place to meet their needs and receive the right services.

Guidance and Support
Through our Intake Counseling services, we help people make informed choices about the types of long-term services and supports (LTSS) they want that fit well with their own preferences, needs, strengths, and values.

For people who are 60 years of age or older who qualified for a long-term approval.
