Maximus performs Pre-Admission Screening and Resident Review (PASRR) Level I screens and Level II evaluations on individuals who are applying to or residing in Medicaid-certified nursing homes. This federally mandated assessment process is used to determine whether the nursing home is the appropriate placement for individuals with mental illness, intellectual disabilities or developmental disabilities. PASRR functions as an essential method to help match individuals with the care and services they need. We also performs nursing facility Level of Care Determinations for all Level II evaluations.

  • Provider Updates and Reminders - Nursing Facility Level of Care Update | Annual Review Process Reminder

    PROCESS UPDATE: NF LOC Documentation

    Beginning this Thursday, December 1, 2022, all Level of Care assessments with a Nursing Facility service type will require the following documents:

    A History and Physical (H&P) that was completed within the last year.

    A History and Physical is a document which includes details of present illness, relevant past history appropriate to the patient's age, drugs, allergies, assessment of body system, conclusion/impression, and plan of care.

    • The H&P must be signed by either an MD, NP, or PA. 
    • ADL Flowsheets from within the last month.
    • You do not need to submit a month's worth of ADL flow sheets. A recent flowsheet from within the last week is an acceptable submission.

    If you do not have these documents, select the I cannot upload all required documents button (see below). Be sure to do this sparingly, as the goal is to have as much information as possible when completing these referrals.

    If you do not submit this documentation, the referral will be placed on hold until you have uploaded them. If you do not upload them within 14 days, the referral will expire.

    REMINDER: Annual Review Process

    Keep in mind that North Dakota DHS will implement a new Annual Review process, beginning on December 14, 2022, for individuals who have a Medicaid payment source (Medicaid, Medicaid Pending, Medicare/Medicaid, and Medicare/Medicaid Pending). 

    Individuals with this payment type will no longer receive unlimited approvals:

    • Every approval will have an end date with a maximum approval of 364 days. 
    • This includes individuals in the past who were previously given approvals with no end date. An end date has been added to all these individuals, regardless of how long they have resided in your facility.

  • SYSTEM UPDATE: Change Coming to AssessmentPro Login Process Tonight, Tuesday, November 1

    Please be aware that this evening, Tuesday November 1, Maximus IT will publish updates to the AssessmentPro system. With these upgrades comes a new way for users to update their passwords. 

    Currently, when users click the reset password button, a link will be sent to the email address you use to log into AssessmentPro, prompting you to create a new password.

    Going forward, instead of receiving a link, a verification code will be sent to your email address, which you will enter on the AssessmentPro site, then create a new password (see below):

  • IMPORTANT PROVIDER UPDATES: Review State Memo | Screening Guidance | Short-Term Expirations vs. Annual Reviews

    Nancy Nikolas Maier, Director North Dakota DHS - Aging Services Division recently sent out a memo to all skilled nursing facility (SNF) administrators, which describes new guidelines regarding a recent settlement agreement with the Department of Justice. As part of this agreement, all Medicaid member residents who are referred for a long-term stay in a SNF or already live in a SNF will be assigned a Case Manager who will make an annual visit to ensure their needs are being met. The DOJ requirement does not start until 12/14/2022 and the annual redetermination dates will be spread across the 12 months of 2023. Maximus will notify facilities of expiring Level of Cares via mail.

    PROCESS REMINDER: Level of Care "Reason for Screening" Guidance

    Nursing facility applicant: Only select this option if an individual is a new applicant for a nursing home. Do not select this option if an individual is already residing in a nursing home.

    (PASRR only) Nursing facility resident who is not currently in an inpatient psychiatric hospital/unit at this time: Only select this option if the individual is currently in a nursing home and the Level of Care is being submitted due to a change in their PASRR condition or if they have never had a PASRR before.

    (PASRR only): Nursing facility resident who is currently hospitalized in a psychiatric hospital/unit: Only select this option if an individual is currently psychiatrically hospitalized and is suspected or known to have a PASRR condition.

    Nursing facility resident who is converting to Medicaid: Only select this option if someone is applying for Medicaid. 

    Nursing facility resident who is requesting retrospective review of this individual's level of care status beginning with a date in the past.

    Nursing facility resident whose approval period is expiring and needs additional time in the nursing facility: Only select this option if the individuals is residing in a nursing home and their previous Level of Care is expiring. 

    Select the appropriate option below based on their payment source.

    Medicaid, Medicaid Pending, Medicare/Medicaid, and Medicare/Medicaid Pending

    • Annual Review: This option should only be selected if the previous LOC approval was for 364 days. 
    • Short-Term Approval Expiration: This option should be selected if the previous LOC approval was for any other period of time (ex. 30, 60, 90, 120 days) 

    Self-Pay, Private Insurance, Medicare
    Nursing facility resident who has had a previous LOC approval, but has had significant medical improvement and the submitting facility is providing an update of that improvement

    Swing bed applicant or swing bed resident applying to Medicaid: Only select this option if the individual is a new Swing Bed applicant OR a current Swing Bed resident applying for Medicaid.

    Waiver, grant, or other service/program applicant: Only select this option if the individual is a new applicant for a waiver service. Do not select this if an individual is already a waiver or service program recipient.

    Waiver, grant, or other service/program recipient whose approval period is expiring and needs additional time in the service/program: Only select this option if the individual is receiving waiver services and their previous Level of Care is expiring.
     

    BEST PRACTICE: Short-Term Expirations vs. Annual Reviews

    When a Level of Care is submitted for individuals in a nursing home due to an expiring past approval, be sure to choose the appropriate option in the Reason for Screening section. The Annual Review option should only be chosen when the most recent Level of Care that is expiring had a 364-day approval period. If the approval period was for any other number of approved days, select the short-term expiration option.

  • System Update: IP Address for AssessmentPro to be Updated on Wednesday, June 15, 2022

    On the evening of Wednesday, June 15, the Maximus IT team will update the IP address for AssessmentPro.com. This change is not expected to affect any existing bookmark or site access for the majority of all system users.

    In the unlikely event this change does affect your ability to access the site after the transition date, please contact your organization’s IT support team. Notify them that you require updated access to the AssessmentPro site and forward the detail below so that they can add the information to their list of approved IP addresses.

  • ND PASRR AND LOC UPDATES: Accessing Locked Provider Resources + New Help Desk Email Address Coming on June 15 + LOC Reminder

    NEW PROCESS: Online Validation for Locked Resources Moving to AssessmentPro

    On the evening of Wednesday, June 15, the validation method for opening locked resource documents on the ND PASRR Provider Tools & Resources site will move from the Legacy login page to AssessmentPro

    The only difference from the previous process is that when you click on password-protected resource links, you will be taken to AssessmentPro to validate your credentials instead of the Legacy System login page that you’ve used previously to gain file access. 
     




    CONTACT UPDATE: New Help Desk Email Address Coming Soon

    Also beginning on the evening of June 15, the North Dakota PASRR Help Desk email address will transition from Ascend-NDPASRR@maximus.com to the simpler NDPASRR@maximus.com. Beginning on Thursday, June 16, all communications emailed to the Help Desk team should be directed to this new address.

    Note: any emails sent to the previous email address after that date will be rerouted to the new inbox to avoid any potential loss of communications submitted.


    PROCESS REMINDER: Nursing Facility Level of Care Determinations for individuals with Medicaid

    Please keep in mind that for individuals who have a payment source of Medicaid, Medicaid Pending, Medicare/Medicaid, or Medicare/Medicaid Pending, all outcomes will now have an end date. The State of North Dakota has requested that anyLOC Determinations that would previously have been given a Long Term Approval, are now given an approval of 364 days.     BEST PRACTICES: Using PathTracker

    AssessmentPro's PathTracker is an important resource in the PASRR process. Review some helpful reminders below for maximizing this helpful tool: 

    • Nursing facilities (NFs) are the only facilities that enter information into PathTracker.
       
    • If your facility has not entered their census, please do so.

    • Please continue to update your PathTracker Census regularly when individuals admit/transfer/discharge/passed away. The state uses this census to schedule HCBS follow-up visits.
       
    • Providers do not need to call from the hospital to update the admitting nursing facility – the accepting NF does that in PathTracker upon NF admission.

    REMINDERS: Tips for Swing Bed Units

    TIP #1
    If you are a swing bed unit, you do not have to submit PASRR Level I Screens. Level I Screens are completed for individuals who are pursuing Nursing Facility admissions.

    TIP #2
    If you are a swing bed unit and you are looking to admit someone with Medicaid, you would only need to submit a LOC.

  • Important State Update: End Date to be Added to Nursing Facility Level of Care Determinations

    Maximus, in partnership with the North Dakota Department of Human Services (DHS) - Aging Services Division has an important update pertaining to Annual Nursing Facility Level of Care Determinations for all Medicaid eligible residents. Please review the memo below and stay tuned for additional communications as well as future webinars as Maximus and ND DHS issue this change:

  • AssessmentPro Update - System Updates Coming 2/22 | New Action Required to Access the Site

    AssessmentPro system security updates are planned for later this evening - Tuesday, February 22, 2022. These routine upgrades will result in changes to some existing system URLs.


    Do I need to take any action?
    Yes. After these updates, any existing AssessmentPro bookmarks you already have saved in your browser will no longer work and should be deleted from your bookmark list. 

    Moving forward, always begin each site visit by typing www.AssessmentPro.com into your browser and proceeding to the section of the site you’d like to view, rather than setting up or using bookmarks. 


    What error will I see if my existing bookmark is broken?
    The bookmark will most likely cause an error that makes it appear that the AssessmentPro site is down - see example below.

  • Helpful Provider Reminders & Updates

    PROCESS UPDATE: "Reason for Screening" on Swing Bed Referral

    If conducting a Level of Care referral for Swing Beds, Providers should select from the bottom two Waiver/Grant choices as the Reason for Screening (see below). We are working on adding Swing Bed to those two options in the future.

    Please keep in mind that the Resources link in the top-right dropdown AssessmentPro menu (see screen capture below) is not currently routing users to the correct URL – the ND PASRR Tools and Resources site. We are working diligently to resolve this issue. In the interim, simply go to a new window and use this site directly to access Provider resources.

    REVIEW: PathTracker Tips & Reminders

    An important reminder that Nursing Facilities are the only facility type that needs to enter PathTracker census information. If you are a medical facility or swing bed, you no longer need to submit tracking.

    To Nursing Facilities: if you cannot enter PathTracker, please reach out to the Help Desk (Ascend-NDPASRR@maximus.com) as soon as possible, as your facility may be improperly registered in the system. In order to update the PathTracker census information, you must have a clinical or non-clinical user role. AssessmentPro Administrators cannot enter PathTracker information.

    FEATURES & BENEFITS: Mastering AssessmentPro
     
    For most system users, the transition will result in few changes to your day-to-day PASRR processes, while maximizing impact with several key benefits and new functionalities. These include:

    • No sharing login information. Everyone MUST have their own login and password. These are legal documents and submitting under another person’s name is fraud. 
    • Support materials. Access training and education under the Resources tab in the dropdown by your name (upper right corner). There are user guides and videos that walk you through how to do most tasks in AssessmentPro.
    • Indicate all facilities. If you work at more than one hospital/nursing facility/agency, you can add those facilities to your profile. One login gets you access to everywhere you work. Refer to the instructions available in the Resources tab if you need help adding facilities to your profile.
    • Review outcomes. You can see outcomes and print determination letters through the system. Look under the Recent Outcomes tab on your home page.

    • Support materials. Access training and education under the Resources tab in the dropdown by your name (upper right corner). There are user guides and videos that walk you through how to do most tasks in AssessmentPro.
    • Indicate all facilities. If you work at more than one hospital/nursing facility/agency, you can add those facilities to your profile. One login gets you access to everywhere you work. Refer to the instructions available in the Resources tab if you need help adding facilities to your profile.
    • Review outcomes. You can see outcomes and print determination letters through the system. Look under the Recent Outcomes tab on your home page.

    In case you or others on your team have yet to sign up for your AssessmentPro account, review the step-by-step breakdown below to choose the appropriate user role and sign up for your system account.

    If you previously created an account but haven't logged into it in the last month or so, please be aware that you will likely need to reset your password. To do so, select the Forgot Password button if your existing login doesn’t work.


    DEFINE: Choose Your AssessmentPro Role
    Your team should determine all necessary AssessmentPro system users who will need access and choose the appropriate role for each (list shown below). Note: The first users set up should always be the facility's designated AssessmentPro (AP) Administrator, as these individuals are responsible for approving for your other facility users.

    Pathtracker and admit all the people in your facility within the application. Be sure to listen for more details on this process during the upcoming webinars listed below.

    • AP Administrator: Responsible for review & approval of all other facility users. 
    • Clinical User: Any individuals who can begin & submit Level I PASRR screen. 
    • Non-Clinical User: Someone who can begin, but can't submit a Level I screen.

    INSIGHTS: Understanding the AssessmentPro Administrator Agreement

    Filling out the Assessment Pro Administrator Agreement form is an important step in the process to get your facility users set up in the system, because without the Administrator role in place, no other users can be set up

    The form is self-explanatory, but one section to note is the signature area. There are two fields for signatures and they each need to be completely filled out. There cannot be any blanks on the form when submitted.

    1. The first signature area is for you to sign, as the organization's AssessmentPro Administrator.
    2. The second signature area is for the Executive Contact. This must be someone who is your supervisor and can confirm that you work in the facility. It cannot be a peer, unless you are the director of your program. Example: A Social Worker's executive contact could be the social work director or supervisor. An RNs executive contact can be the Director of Nursing or their supervisor. If you are the Director of Nursing, you can get the Director of Social Work or the head of the Nursing Home to sign off as the Executive Director. 

    You cannot sign both areas yourself. If this occurs, the form will be rejected.

    After you've begun the training process in the upcoming webinars mentioned above, setting up a facility account in AssessmentPro and choosing appropriate system user roles for your team members will then be two important next steps. Below is a brief preview of the various roles available - the most important of which are AssessmentPro (AP) Administrators. These should be the first accounts to sign up, as these individuals will then review and approve all other users who sign up for system accounts from your facility. 

    AssessmentPro User Roles

    AP Administrator  
    This role must be set up first. Responsible for review and approval of all other facility users. It's recommended that multiple AP Administrators be selected to ensure continuity. 

    Clinical User
    This role applies to individuals who can begin and submit a Level I screen.
     
    Non-Clinical User
    Applies to individuals who can begin but cannot submit a Level I screen.

    *You can sign up for multiple roles. For example, an AP Administrator can also be a Clinical User.

    Action Item: Ahead of the training webinars, begin thinking about who in your team should be set up as AP Administrators and what role(s) you should have. Make sure that everyone involved in the PASRR process is aware that they will need to sign up for a user role once the ND PASRR Training team shares instructions to complete that process.

    PROCESS: What Changes Can I Expect with AssessmentPro? 

    The most important reason behind the advanced tools and features in AssessmentPro is the generation of assessments that paint a more well-defined picture of each individual's service needs. From the North Dakota providers perspective, there will be an updated user interface but only be a few changes to their existing PASRR workflow, including several beneficial updates from the current system:

    1. Simplified Processes
      Providers who work for multiple facilities will only need one AssessmentPro login to access the system. You will simply toggle to the appropriate facility as necessary.
       
    2. Digital Upload of Documents
      Submission of materials becomes easier, with the option to digitally upload documents directly into the system instead of the currently required fax process.
       
    3. Printing Letters
      AssessmentPro allows the option to easily print letters for individual/guardian/physician and hand out to them. 
       
    4. Expanded Resources
      Providers will have access to a large array of detailed system training guides, recordings, and other support materials behind the AssessmentPro system login.
       

    NEXT STEPS: New Learning Opportunities Planned
     

    In the final few weeks ahead of the AssessmentPro migration, the ND PASRR training team will offer a comprehensive webinar series covering a wide range of topics to make sure your facility is ready to begin using the system on day one. These planned sessions include:
     

    Transition Training | September 1 - 2
    These sessions will focus on what is changing, an overview tour of AssessmentPro, understanding the various system roles available, and how to initiate your user registration. 
     

    Completing of Level I & LOC | September 15, 16, 17
    Attendees will walkthrough completing a Level I and Level of Care (LOC) - including how to submit screens, locate outcomes, submit tracking, and other important points of AssessmentPro.
     

    AssessmentPro Pre-Launch Summary | September 22 - 23
    This informative session will cover several key topics including a review of AssessmentPro roles, system registration, tour of features, submitting screens and tracking, as well as a walkthrough of all available support resources. 
     

    REMINDER: Hybrid PASRR Assessment Model Now in Place

    As shared in a recent communication, the North Dakota PASRR program transitioned on July 21 to a hybrid assessment model, which includes the option of completing assessments in-person. Decisions on assessment format, either in-person or virtual - performed through HIPAA-compliant Zoom account, are determined by the preference of each facility on a case-by-case basis and are scheduled to began on Wednesday, July 21.
     

    What to expect with an in-person assessment

    Assessors are taking the necessary steps to prepare for this transition in the safest manner possible, relying on CDC guidelines and best practices. 

    • Assessors will call in advance to arrange the in-person visit.
    • Steps to be taken include the stringent use of Personal Protective Equipment (PPE) during each visit.
    • Assessors will work directly with the Maximus support team to submit health assessment forms on any days where an in-person interview is planned.
